South & East Leeds GP Group is seeking Care Coordinators to join our PCN, helping to reduce health inequalities and support meeting PCN and practice targets. They will identify, engage with and proactively coordinate personalised care for the most vulnerable people in our community.
They will work closely with practice and PCN staff and with other practices as a team, sharing learning and best practise across the PCN to improve engagement and self-management for patients and carers.
