The Role
As an Adult Social Care Practitioner you will support residents across their homes, hospitals, and care settings, assessing needs and coordinating tailored support that promotes independence and wellbeing.
Your Role Will Involve:
- Assessing and supporting adults with care needs in line with the Care Act 2014
- Managing a caseload and working collaboratively with individuals, families and professionals
- Coordinating services to promote independence and wellbeing while ensuring compliance with legislation, policy and budgetary frameworks
- Safeguarding, risk management, support planning and maintaining accurate records
- Contributing to team performance and upholding council values and governance standards
