We have an exciting opportunity for a Social Care Practitioner to join us! This is a fullātime, permanent role based in the Town Hall in Maidenhead with some hybrid working, and a salary of Ā£24,895 ā Ā£28,132 per annum. This is a great opportunity for anyone wanting to work in Adult Social Care.

Role Overview
As an Adult Social Care Practitioner, you'll play a vital role in supporting residentsāwhether in their own homes, hospitals, or care settingsāby assessing their needs and coordinating tailored support that promotes independence and wellbeing.
Responsibilities
- Assessing and supporting adults with care needs in line with the Care Act 2014.
- Managing a caseload and working collaboratively with individuals, families, and professionals.
- Coordinating services that promote independence and wellbeing.
- Key responsibilities include safeguarding, risk management, support planning, and ensuring compliance with legislation, policies, and budgetary frameworks.
- Contributing to team performance, maintaining accurate records, and upholding the council's values and governance standards.
