Sedgefield 1 Primary Care Network is seeking a Social Prescribing Link Worker to join our growing multi-disciplinary team. You will support patients in their health and wellbeing by connecting them with groups and services, promoting prevention and lifestyle changes, and working with wider health, social care and voluntary networks.
The post holder will travel between practice sites and patient homes, build local networks, signpost patients to sources of support, and help practices meet referral.
