An exciting opportunity has arisen for a Health & Wellbeing Coach to join our integrated personalised care team. You will join our growing PCN multi-disciplinary workforce which includes Social Prescribing Link Workers, Clinical Pharmacists, Pharmacy Technicians, First Contact Physiotherapists and Mental Health Nurses.
The Health & Wellbeing Coach will be responsible for delivering group and one to one sessions on all aspects of health and wellbeing. The coach will play a critical role in engaging patients and use health coaching techniques to support them to take an active role in their health. You will work in a variety of venues in general practice and the community. Teamwork, flexibility, enthusiasm, and the ability to prioritise tasks effectively are essential skills.

Durham Coast Primary Care Network is an innovative, dynamic PCN combining 4 local GP Practices based in the Easington area, with a population of over 33,000 patients. We are continually seeking new ways to support and improve local healthcare services to our patient population.
We are looking for an individual who will play a role in engaging patients and use health coaching techniques to support them to take an active role in their health and wellbeing. The Health and Wellbeing Coach is responsible for delivering group and one to one sessions to improve patient health outcomes. This is a new role in our PCN and as such the coach will be able to help shape and develop the role based on their particular passion such as healthy eating, physical activity, improving mental health etc.
The health and wellbeing coach will work closely with those of low to medium complexity who will usually have 1 or more long term conditions. The post holder will have a key role in helping to raise the local population's awareness of the support groups and opportunities available to assist them in achieving their health and wellbeing goals. As a whole, your role and skills will support and encourage the prevention of developing further illness, or the deterioration of existing long-term conditions.
KEY RESPONSIBILITIES- Identification of people with long term conditions and low knowledge skills and confidence to manage their health and wellbeing.
- Providing support (clinical or non-clinical) to a cohort of patients who will benefit from proactive health management and care.
- Playing a health and wellbeing coaching role; teaching and supporting patients/carers to understand and manage their own conditions.
- Supporting the development of personalised patient care plans, liaising with the practice team, patient/carer and the complex care team as appropriate.
- Proactively supporting practice targets for number of patients who have seen a health coach.
- Coordinating patient visits to primary care amongst the primary care team.
- Map and connect community activities/resources at a locality level.
- Support the delivery of community based public health initiatives.
- Build and maintain strong links with the voluntary sector.
- Contribute to keeping the community directory up to date for their allocated area.
- Managing the effective use of available resources and services.
- Understand when it is appropriate or necessary to refer people to other health professionals/agencies.
- Ensure the patient record is accurately updated and maintained.
- Promote the service within the Primary Care Network.
- Communicate effectively with colleagues, patients and carers.

FURTHER AREAS OF RESPONSIBILITY- Health and Safety/Risk Management: The post-holder must comply at all times with the Network Health and Safety policies.
- Information Governance: You have a duty to process sensitive information judiciously and lawfully.
- Performance Appraisal and Personal Development Planning: Each employee receives an Annual Performance Appraisal and a Personal Development Plan.
- Statutory and Mandatory Training: All required training must be completed within the induction period.
- Safeguarding Children and Vulnerable Adults: You are accountable to ensure that you know how to respond when you are concerned for the safety of a child, young person or vulnerable adult.
- Infection Prevention and Control: All employees are expected to follow consistently high standards in the prevention and control of infection.
- Special Working Conditions: The post-holder is required to travel independently between practice sites.
