South Coast Nursing Homes is an established and successful, family owned and managed, care home company based in Worthing, West Sussex. We currently operate 8 homes (soon to be 9) across East and West Sussex. We have 359 beds and will be adding a further 55 beds to our portfolio with the opening of a new Nursing home in central Worthing within the next 2 months. Each registered home offers the highest standards of resident care and comfort, 24 hours – day and night, for long and short-term stays.
Reporting to the Home Manager, the Welfare Liaison is responsible for supporting the entire care team to meet the needs of residents’ welfare and well-being. The Welfare Liaison must have exemplary communication skills and a keen eye for detail. The role encompasses seeing each resident as individuals and formulating the structure in which to meet their holistic requirements for living well, including the recognition that family members or friends are also integral to this process and so collaboration between external and the internal elements are key to success of the role.
Hours: Full-Time at 40 hrs p/w - 9am - 5pm Monday to Friday.
As a Welfare Liaison you will be:
- Liaison with resident, family and friends, and care team
- Create robust documentation for every resident, detailing who they are, their specific needs and preferences
- Co-ordinate safe and seamless admissions and discharges/end of residency
- Identify, arrange and coordinate all external and internal health and sensory appointments
- Carry out assessments and ensure implementation of outcomes
- Audits
- Carry out and share pertinent documentation regarding residents to other health teams
- Assist with documents and procedure pertaining to fire regulations
- Ensure that all regulatory and statutory requirements are met and company policies and procedures are adhered to.
- Signposting relatives and friends to external agencies for support
More about you:
- NVQ3 or above in Health and Social Care
- Experience in a similar role
- Passionate about making a difference on a daily basis
- Previous experience in geriatric, frailty, end of life and dementia care.
- Kind caring, positive approach
- Good reporting skills both verbal and non-verbal
