We are looking for a reliable and motivated Care Coordinator to assist our pharmacy team. You will support pharmacists and pharmacy technicians in providing safe and effective medicines services to our patients. This is a fixed part-time role, we are looking for someone who could support our existing team Monday to Friday.

Main duties of the job:

  • Supporting the smooth day-to-day running of the pharmacy team.
  • Process digital and paper prescriptions.
  • Liaise with patients, pharmacies, care homes and clinical teams to support effective medicines management.
  • Work alongside colleagues or independently to deliver project work relating to medicine optimisation.
  • Recall patients for annual monitoring.
  • Commence electronic repeat dispensing (eRD) where appropriate.
  • Identify appropriate patients and book appointments for services such as medication reviews, asthma clinics, blood pressure checks, and other NHS health consultations.
  • Maintaining patient records and updating pharmacy systems.
  • Following NHS safety and confidentiality procedures.

Sound PCN is a network of 5 practices providing proactive and coordinated care across a population of 42,000 patients in the North West area of Plymouth. We have a strong focus on health promotion and personalised care, supporting patients to make informed decisions about their health and social care. Sound PCN have been early adopters of new models of NHS care, utilising the diverse skill mix offered by FCPs, paramedics, nurse practitioners, social prescribers, pharmacy technicians and care coordinators.

Job responsibilities:

  • To assist Practice Management and other non-clinical staff concerning administrative aspects of medicines management as appropriate.
  • To optimise medicine management, resolve queries where possible and produce prescriptions for clinical review and signature.
  • Optimisation of repeat medication, commencing repeat dispensing and synchronisation of medication.
  • Ordering of required monitoring i.e., blood tests, blood pressure, height, weight etc. for medicine/condition monitoring.
  • Assist pharmacy team with applicant requests from District Nurses.
  • Co-ordinate medical device prescription requests, including requests for catheters, nutritional supplements/feeds, and stoma care products.
  • Work alongside colleagues or independently to deliver project work relating to medicine optimisation.
  • Liaise with patients, pharmacies, care homes and clinical teams to support effective medicines management.
  • To work in conjunction with the nominated Clinicians who support Nursing Homes in the area.
  • Provide information to patients regarding medicines and prescribing changes, initiating further support from other healthcare professionals where appropriate.
  • Support the PCN to deliver on QIPP agenda, QOF, IIF (Investment and Impact Fund), DMOS (Devon Medicines Optimisation Scheme) and other locally commissioned.

Person Specification:

Qualifications:

  • GCSE A-C in English and Maths (or equivalent).
  • Level 2 NVQ Certificate in Pharmacy Service.

Experience:

  • Basic IT skills with the ability and willingness to learn new pharmacy systems and software.
  • Experience of working in primary or secondary care.
