We are seeking compassionate, motivated District Nurses to join our Clients community nursing team. This role is ideal if you are passionate about delivering high-quality, patient-centred care in a community setting, supporting individuals to remain independent in their own homes. You will work autonomously as well as part of a multidisciplinary team, providing skilled nursing care to patients with a range of complex physical and long-term conditions.
Main duties of the job:
- You will visit adult patients in their own home who have an identified nursing need including those with a long-term condition, complex care needs and needing care at home and the end of life.
- You will use an electronic patient held record system, EMIS as a record of care and all nurses are provided with a work iPhone and iPad for mobile working.
- The community nurse will use a range of nursing skills including venepuncture, wound care, catheterisation, syringe drivers, end of life care and symptom control.
- You will clinically manage a team of nurses and health care assistants and be accountable for a caseload of patients ensuring the effective, efficient, safe and high quality nursing assessment, treatment and delivery, and evaluation of nursing care for patients and carers.
- You will be the named District Nurse for a GP practice and will work closely with GP's, Practice Nurses, social care, local hospitals, specialist nurses, therapists, adult social care, hospices and palliative care teams, mental health services, the voluntary sector and others to help individuals manage their own conditions, maintain their independence and remain at home wherever possible.
