We are seeking an experienced and dedicated Community Adult Complex Care Nurse. This rewarding role focuses on supporting clients with complex medical conditions such as neurological conditions, spinal injuries, or trauma. You will play a pivotal role in managing clients' care, leading and delivering person-centred training to carers, and ensuring the delivery of high-quality care in the community.
The Role:
- Provide clinical oversight for clients who have complex care needs, enabling them to live within their own home in the community.
- Assess clients’ needs and develop person-centred care plans.
- Provide a comprehensive competency-based training programme for the care workers assigned to the clients, which may include invasive and non-invasive ventilation, tracheostomy management and care, epilepsy management, enteral feeding, and medication via PEG.
Your Responsibilities Will Include:
- Comprehensive Care Planning: Designing, implementing, and reviewing personalised care plans to ensure every client’s unique needs are met effectively and safely.
- Training and Education: Delivering hands-on training to carers, equipping them with the knowledge and confidence to manage clients; tracheostomy care, ventilation, spinal/AD, PEG feeding, neurological conditions, and medication administration.
- Clinical Expertise: Providing high-quality nursing care while adhering to best practices in complex care.
- Collaboration: Working with multidisciplinary teams to provide holistic, person-centred care, liaising with therapists, GPs, and other healthcare professionals.
- Ongoing Support: Acting as a key point of contact for clients, families, and carers, offering guidance, emotional support, and advocacy.
About You
We are looking for an experienced, caring, empathetic, and compassionate nurse with:
- Qualifications: Adult Registered Nurse with a valid NMC registration.
- Experience: Significant experience in adult critical care or in a community complex care setting, with the ability to manage tracheostomy care, ventilation, PEG feeding, neurological and spinal conditions, and complex medication regimens.
- Skills: Strong teaching and mentoring abilities to effectively train carers.
- Attributes: A calm, empathetic, and client-centred approach, with excellent organisational skills and the ability to work independently in the community.
- Driving License: A full UK driving license and access to a vehicle are essential.
