Your Purpose (aka why you matter here): You’ll oversee the entire assessment process—from admission to ongoing care—making sure everything is accurate, timely, and meets all guidelines. You’re the bridge between data, care, and outcomes.
What You’ll Be Doing (aka your daily wins):
- Oversee and complete all MDS assessments (PPS, quarterly, annual, significant change—you know the drill)
- Keep every assessment on schedule, accurate, and compliant
- Lead and support weekly PDPM meetings for skilled residents
- Track and monitor PDPM tools (yes, including all the important ones)
- Analyze Quality Measures (QM) and QAPI/QASP data—and help us improve outcomes
- Assess all new admissions and determine level of care
- Communicate resident care needs across all departments
- Coordinate interdisciplinary teamwork for MDS completion
- Keep MDS schedules organized with accurate reference dates
- Handle data entry, submissions, corrections, and tracking like a pro
- Schedule and lead care conferences (fully prepared, of course)
- Help build and revise care plans that are individualized and meaningful
- Evaluate outcomes and adjust care plans when needed
- Manage level-of-care changes and keep everyone informed
- Complete and submit required forms per state regulations
- Train and mentor new MDS Coordinators (yes—you’re the expert!)
- Protect resident confidentiality at all times
What You Bring to the Team:
- Active RN (BSN preferred) or LPN license in good standing
- Graduate of an accredited nursing program
- Long-term care experience (you understand the environment)
- 3+ years of supervisory or leadership experience preferred
- Strong knowledge of MDS, PDPM, and care planning
- Attention to detail that others wish they had
- Ability to manage deadlines, data, and teamwork all at once
Skills & Tools:
- Comfortable with Microsoft Office
- Experience with PointClickCare (PCC) is a big plus
- Keeps up with continuing education and licensing requirements
