Due to expansion within our nursing team we are recruiting for a part time Practice Nurse to join our friendly team at Sonning Common Health Centre. The vacancy is for 20-30 hours per week, Monday to Friday. Exact working patterns and hours will be discussed in interview. It is a diverse and interesting role working with an established nursing team and wider multidisciplinary team.
Responsibilities
- Provide high quality evidence-based nursing care
- Support GP clinics
- Childhood and adult immunisations and other injections under Patient Group Directions
- Provide wound management including both chronic and post-operative wounds
- Carry out Health Checks in conjunction with national, local and practice initiatives
- Treatment room responsibilities
- Develop, implement and embed health promotion and well-being programmes
- Provide routine nursing care to patients as required in accordance with clinical base evidence, NICE
- Provide travel medicine services, within scope of knowledge
- Request pathology services as necessary
- Process pathology results as required
- Maintain accurate clinical records in conjunction with extant legislation
- Maintain accurate clinical documentation in accordance with NMC and Practice Standards
- Chaperone patients where necessary
- Assist GPs with minor surgery when required
- Prioritise health issues and intervene appropriately
- Support the team in dealing with clinical emergencies
- Implement vaccination programmes for adults and children under Patient Group Directions
- Work with external services and agencies to ensure the patient is supported appropriately, vulnerable patients etc.
- Delegate clinical responsibilities appropriately, ensuring safe practice and the task is within the scope of practice of the individual
- Support the clinical team with all safeguarding matters, in accordance with local and national policies
- Deliver opportunistic health promotion where appropriate

- Post graduate diploma or degree in Primary Care
- Clinical knowledge and skills
- Wound Care / Removal of sutures
