We are looking for caring and motivated individuals to join our friendly and supportive GP practice team. This is an exciting opportunity for someone who is passionate about delivering high-quality patient care within a busy primary care setting.

Main duties of the job

  • Deliver high-quality, compassionate, and person-centred care under the supervision of registered nurses.
  • Assist the practice clinical team in the provision and delivery of prescribed programmes of patient care.

Job responsibilities

  • Clinical Responsibilities: To carry out ECG recording on patients as requested, in line with practice protocols, ensuring that the output and results are passed to the appropriate clinician for review.
  • Provide phlebotomy services for patients as per practice protocols.
  • To carry out NHS Health Checks as per practice protocols which include the internal & external quality control checks on the equipment for the St Chads site, flagging any discrepancies to the Nurse Manager immediately.
  • Assist with the GP with Minor Operations and Long-Acting Reversible Contraception clinics.
  • Administer immunisations, medication and vaccinations under Patient Specific Directions as directed.
  • Support the practice nurses in delivering delegated wound care, including removal of sutures and clips, wound dressings following a wound care plan, and compression bandaging under supervision of a registered nurse mentor.
  • Support the practice in delivering long term conditions care for patients as assessed competent to do so, including diabetic foot checks and respiratory diagnostic testing.
  • Provide ear checks to patients prior to their audiology appointment.
  • Undertake specific clinical activities for named patients that have been delegated and taught specifically in relation to that individual.
  • Provide chaperoning duties for patients and/or clinicians as requested, ensuring that chaperone training is up to date and practice protocols are followed.
  • Process and manage laboratory samples to send off to the pathology lab as requested by GPs/Nurses.

Administration and professional responsibilities:

  • Participate in the administrative and professional responsibilities of the practice team.
  • Ensure accurate and legible notes of all consultations and treatments are recorded in the patients records.
  • Ensure that medical records are completed at time of consultation and are accurate and that confidentiality is respected inline with surgery policy.
  • Ensure the clinical computer system is kept up to date with accurate details recorded of any consultation or patient contact.
  • Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.
  • Ensure collection and maintenance of statistical information required for regular and ad hoc reports.
  • Order general clinical surgery supplies under the direction of the nurse manager.
  • Provide the service of restocking clinical rooms and cupboards and ordering needed items of nursing supplies for the St Chads site.
  • Supervise junior staff to ensure stock levels are maintained and support junior staff escalating any concerns with performance to associate lead nurse and/or nurse manager.
  • Be responsible for maintaining the vaccine/cold chain storage and the monitoring of vaccine fridges.
  • Prepare and maintain the clinical environment and equipment in own clinical room before, during and after patient care interventions.
  • Assist in the assessment and surveillance of patients health and well-being.
  • Help raise awareness of health and well-being with patients.
  • Assist with the collection and collation of data on needs related to health and well-being.
  • Assist in seasonal and special projects as requested e.g. flu and covid vaccination campaign.
  • Take part in the appraisal and training structures of the practice.
  • Attend and participate in any team or whole practice meetings when required.
  • Provide administrative support to the nurse manager as requested.
  • Other duties as requested from time to time.

Person Specification

  • Knowledge & Skills: Blood pressure pulse, temperature, height weight, ECGs, familiar with TPP System One or Emis web, wound care and removal of sutures, health promotion, diabetes care - foot checks, chronic disease management - pre-assessment checks, automated Doppler assessments of ABPI, measure for compression hosiery, assisting in minor ops or coil clinics.
  • Qualifications: Maths and English minimum Grade C or an equivalent numeracy/literacy certificate, NVQ 3 in Health & Social Care or an equivalent recognised qualification in healthcare, minimum of two years healthcare assistant experience within a primary care setting.
  • Disclosure and Barring Service Check: This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
