Why This Role is Different: Join our brand-new Family Practice at 775 South Main Street and help shape the future of patient-centered care from day one. Our team-based model brings providers, nurses, and support staff together in a collaborative environment where every voice matters and every role contributes to exceptional outcomes. As a member of our innovative care team, you'll practice at the top of your license through a blend of hands-on patient care, chronic disease management, live-answer triage, nurse-provider co-visits, and care collaboration with our ancillary services. This role offers a unique opportunity to build meaningful patient relationships, support population health initiatives, and play an active role in shaping workflows and care delivery within a newly established family practice.
Position Summary: M - F 7:30 am - 7pm. Flexible Schedule with 4 x 10 hour days or 5 x 8 hour days. Delivers care to patients and families with needs ranging from basic to complex utilizing the nursing process. Creates a therapeutic and caring environment for patient and families. Practices in a manner consistent with the Elliot Hospital Interprofessional Practice model; the ANA’s Social Policy Statement, Scope and Standards of Practice, and the Nursing Code of Ethics; and relevant specialty standards of practice.
Key Responsibilities
- Embraces and promotes change to support optimal patient outcomes and an effective professional practice environment.
- Involves patient/family in goal setting and planning and communicates that plan of care to patient and family for input; and to colleagues to ensure seamless care across shifts and settings.
- Utilizes current evidence and relevant national/specialty standards to guide practice and/or performance and identifies areas for improvement.
- Demonstrates competent clinical practice utilizing clinical decision making and critical thinking skills.
Education/Experience/Licensure
- Education: Graduate of an accredited school of nursing. Bachelor's of Science in Nursing preferred.
- Licensure: New Hampshire Registered Nurse or Compact State Registered Nurse License.
- Technical Certification: Ability to obtain BLS qualification during orientation period and maintain during employment. ACLS preferred.
