Social Work Choice UK Agency are recruiting for a local authority looking for a care coordinator to work with adults service Outreach. This role is for a care coordinator, on scale 4 point 7.

The role of this worker would be to:

  • Monitor support visits and ensure they are logged
  • Develop staff rotas using the call monitoring system
  • Liaise with staff and send out rotas
  • Monitor that visits are being completed
  • Manage team email and respond to referrals
  • Log referrals onto a spreadsheet and track with team leaders

Working hours: 18 hours weekly on a rolling rota basis.

Rota:

  • Week 1 - Tuesday 9-5pm, Wednesday 9-4.30pm, Thursday 9-1.30pm
  • Week 2 - Monday 9-12.45pm, Thursday 12-5pm, Friday 9-1pm

Duties & Responsibilities:

  • Support the day-to-day financial, clerical and administration functions
  • Maintain effective and efficient administrative and information management systems
  • Provide administrative support and attend relevant meetings
  • Use relevant databases for data input and interrogation
  • Deal with straightforward customer enquiries and follow through to resolution
  • Ensure data and records are maintained in accordance with agreed standards
  • Provide a customer care focused reception service
  • Identify difficulties visitors/service users may have in making enquiries
  • Contribute ideas and make recommendations in service meetings
  • Develop and maintain knowledge of relevant policies, procedures and legislation
  • Work as part of a project team and assist in the delivery of work projects
  • Advise and train colleagues in specialist activities
  • Handle service users’ monies where required
  • Use the Council’s electronic procurement system (SRM)
  • Assist managers with financial processes
  • Attend confidential service specific meetings
  • Resolve problems that require discretion
  • Assist less experienced colleagues with complex enquiries
  • Organise and prioritise own workload
  • Assist managers in Health and Safety checks/audits
  • Manage own performance to meet goals and deadlines
  • Participate in performance management and development discussions
  • Undertake any other reasonable duties commensurate with the grading of the post

Qualification & Experience:

  • NVQ/QCF or equivalent Level 2 in Social Care or IT
  • Experience of working in an adult social care environment
  • Experience of working with an electronic rota allocation system
