Social Personnel is advertising a Social Worker role for Durham County Council. The Next Steps Team is a newly established service supporting young people aged 16β17 as they prepare for adulthood. The Social Worker will work alongside young people to develop Pathway Plans, set aspirational goals, and ensure a smooth transition into care leaver services.
Key Responsibilities
- Manage a caseload of young people aged 16β17, providing consistent support and guidance.
- Work collaboratively with young people to co-produce Pathway Plans that reflect their aspirations and needs.
- Support the transition from care to independence, ensuring access to appropriate services and opportunities.
- Liaise closely with the Care Leavers Service and Children in Care Service to provide continuity of support.
- Utilise wraparound services such as Staying Close, Peer Mentoring, and emotional wellbeing support to enhance outcomes.

Essential Skills & Experience
- Qualified Social Worker registered with Social Work England.
