Social Personnel is advertising a Social Worker role for Durham County Council. The Next Steps Team is a newly established service supporting young people aged 16–17 as they prepare for adulthood. The Social Worker will work alongside young people to develop Pathway Plans, set aspirational goals, and ensure a smooth transition into care leaver services.

Key Responsibilities

  • Manage a caseload of young people aged 16–17, providing consistent support and guidance.
  • Work collaboratively with young people to co-produce Pathway Plans that reflect their aspirations and needs.
  • Support the transition from care to independence, ensuring access to appropriate services and opportunities.
  • Liaise closely with the Care Leavers Service and Children in Care Service to provide continuity of support.
  • Utilise wraparound services such as Staying Close, Peer Mentoring, and emotional wellbeing support to enhance outcomes.
  • Maintain accurate case records and ensure compliance with statutory requirements and council policies.

Essential Skills & Experience

  • Qualified Social Worker registered with Social Work England.
  • Experience of working with young people in care or preparing for independence.
  • Strong knowledge of legislation and guidance relating to children in care and care leavers.
  • Excellent communication, relationship-building, and planning skills.
  • Ability to work collaboratively within a multi-agency environment.

Desirable

  • Experience of Pathway Planning and supporting transitions to adulthood.
