Role: Social Worker – Next Steps Team

Purpose of Role
Social Personnel is advertising a Social Worker role for Durham County Council. The Next Steps Team is a newly established service supporting young people aged 16‑17 as they prepare for adulthood. The Social Worker will work alongside young people to develop Pathway Plans, set aspirational goals, and ensure a smooth transition into care leaver services.
Key Responsibilities
- Manage a caseload of young people aged 16-17, providing consistent support and guidance.
- Work collaboratively with young people to co‑produce Pathway Plans that reflect their aspirations and needs.
- Support the transition from care to independence, ensuring access to appropriate services and opportunities.
- Liaise closely with the Care Leavers Service and Children in Care Service to provide continuity of support.
- Utilise wraparound services such as Staying Close, Peer Mentoring, and emotional wellbeing support to enhance outcomes.
- Maintain accurate case records and ensure compliance with statutory requirements and council policies.
Essential Skills & Experience
- Qualified Social Worker registered with Social Work England.
- Experience of working with young people in care or preparing for independence.
- Strong knowledge of legislation and guidance relating to children in care and care leavers.
- Excellent communication, relationship‑building, and planning skills.
- Ability to work collaboratively within a multi‑agency environment.
Desirable
- Experience of Pathway Planning and supporting transitions to adulthood.
- Knowledge of local services and support networks available to care leavers in Durham.
