Social Personnel are recruiting for 2x Social Workers for a Transitions Team. This position is to work remotely for the majority but you are required to work one day in the office.

Essential Experience:
- Experience in a Transitions Team
- Experience working in Adults Disabilities
- Social xgikmsk Work England Registration
- Degree in Social Worker
