An exciting new opportunity has opened to join a contract in Lewisham, providing support across two properties for residents and participants with acute and/or complex mental health needs. These individuals receive community-based care as an alternative to hospital inpatient treatment. Your focus will be on rehabilitating and reintegrating them, helping them achieve their personal goals and reach a level of recovery that allows them to return home or transition into supported housing.

The Integrated Community Rehabilitation Service offers a community-based alternative to avoid admission into a rehabilitation unit.

This service operates across two properties in Lewisham-one with 8 self-contained rooms and the other with 6, both located within close proximity to each other. Each site is staffed 24/7 by at least two Mental Health Recovery Workers (MHRW), with Shift Leads providing additional support to the management team. The properties are just a short walk from one another and are conveniently located near public transport, including Bellingham, Catford, and Catford Bridge train stations, as well as nearby bus stops.

Shift Pattern: 37.5 hours per week, Rota based Monday to Sunday between 08:00 and 20:30, * Develop and maintain therapeutic relationships with residents and participants, providing emotional and practical support to facilitate their recovery journey.

  • Co-produce and implement tailored support plans for residents, ensuring their individual needs are met through a trauma-informed and recovery-focused approach.

  • Act as a key worker or link worker, contributing to support plans, risk assessments, and regular reviews for assigned participants.

  • Provide advice, information, and guidance on mental health, medication, and therapeutic interventions, supporting residents in understanding and managing their well-being.

  • Lead and participate in reflective learning sessions, promoting continuous development within a Psychologically Informed Environment (PIE).

  • Take shift lead responsibilities on rotation, ensuring effective day-to-day service delivery and managing follow-up actions for residents.

  • Manage housing responsibilities, ensuring safety, cleanliness, and maintenance of properties, while supporting staff and residents.

  • Complete safeguarding referrals, attend appointments, and provide administrative support, maintaining records in line with statutory and organisational policies.

Other responsibilities than those described above may be required to be undertaken from time to time and will be expected to be performed to as long as it is within the capability and level of the position.

Your strong communication skills, empathy, and understanding of mental health challenges help you navigate sensitive situations with professionalism and respect for confidentiality.

What are we looking for?

  • Previous experience working with people with multiple complex background/needs, mental health, substance misuse, alcohol, and behaviours that can be challenging

  • Previous experience creating co-produced support plans, key working, and care planning, providing appropriate interventions for service users

  • IT Proficiency, ability to learn new software programs, basic Microsoft experience

What would we like, but not essential?

  • Understanding and/or practical application of key legislation regarding social care, housing, criminal justice, and mental health

  • Ability to Recognise signs of deteriorating mental health and initiate appropriate interventions to prevent crisis

  • Appropriate Professional Qualification: Psychology Degree, NVQ, QCF or Level 3 Diploma in Health and Social Care or Community Justice, or professional equivalent

  • 25 days annual leave, increasing with the length of service
