This South London Authority are looking for a Qualified Social Worker to work within The Assessment & Intake Team.
Details of the role are:
- Main duties will be holding a case load, completing Care Act Assessments, undertaking unscheduled reviews of care, completing carers assessments, presenting packages of care to the funding panel for approval, using community resources to support service users and their families to maximise their independence and to ensure that all interventions are strength based.
- You would also be responsible for completing Mental Capacity Assessments and Best Interest Decision making, arranging care home placements from the community, stabilising complex case issues ready for transfer to the longer term team and undertaking the initial concern stage of Adults Safeguarding.
- The role includes the provision of a short-term intensive assessment period, assessing the person’s ability to maximise their independence and to provide a robust assessment that measures the amount of ongoing support needed.
- In the role you will liaise and work closely with partner agencies such as the NHS, ICB, housing and community voluntary agencies in addition to working closely with the Occupational Therapy staff within the team.
- You will need to be skilled in thinking creatively in meeting the needs of service users and informal carers and will be required to utilise strong communication skills in building relationships and trust with those whom you work.
