The Role

We are seeking an experienced and highly organised Complex Care Coordinator to join our growing team in Hull. You will play a key role in managing care packages, supporting clinical teams, and ensuring the highest standards of care delivery for our service users and managing rotas.

Key Responsibilities

  • Coordinate and manage complex care packages within the community
  • Liaise with families, healthcare professionals, and multidisciplinary teams
  • Develop, review, and update person-centred care plans
  • Ensure compliance with regulatory standards and company policies
  • Support recruitment, onboarding, and supervision of care staff
  • Monitor care delivery, quality, and client satisfaction
  • Manage rotas and ensure adequate staffing levels
  • Respond to incidents and implement improvement plans where required

Requirements

  • Previous experience in a complex care or community care coordination role
  • Strong understanding of clinical needs such as tracheostomy care, ventilators, PEG feeding, and neurological conditions
  • Excellent organisational and communication skills
  • Ability to manage multiple priorities in a fast-paced environment
  • Knowledge of CQC regulations and compliance standards
  • Full UK driving licence (preferred)
