As a Care Coordinator, you will play a pivotal role in the day-to-day coordination and scheduling of care services, ensuring that our clients receive the very best standard of care while supporting and guiding a dedicated team of care workers. Key Responsibilities

As a Care Coordinator, your responsibilities will include, but are not limited to:

  • Coordinating and scheduling care worker rotas to ensure full care coverage across the service at all times
  • Acting as the first point of contact for clients, their families, and care workers regarding any queries, concerns, or changes to care plans
  • Conducting initial client assessments and regularly reviewing and updating individual care plans in line with changing needs
  • Monitoring the quality and delivery of care services to ensure compliance with CQC regulations and company policies
  • Supporting the recruitment, induction, and ongoing training and development of care workers
  • Managing and responding to any incidents, complaints, or safeguarding concerns in a timely and professional manner
  • Building and maintaining strong, trusting relationships with clients, families, healthcare professionals, and external agencies
  • Completing accurate and detailed administrative tasks, including maintaining up-to-date client and staff records
  • Participating in on-call duties as required to support the smooth running of the service
  • Assisting the Registered Manager in ensuring the service meets all regulatory and contractual requirements
Requirements

The ideal candidate will demonstrate the following skills, qualifications, and experience:

  • Previous experience working within a domiciliary care or home care setting is highly desirable
  • Strong organisational and time management skills with the ability to prioritise a demanding workload
  • Excellent communication and interpersonal skills, both written and verbal
  • Proficiency in care rostering and scheduling software or a willingness to learn new systems
  • A good understanding of CQC regulations and compliance standards within the care sector
  • Knowledge of safeguarding procedures and a commitment to protecting vulnerable adults
  • A compassionate, empathetic, and person-centred approach to care coordination
  • Ability to work effectively both independently and as part of a wider team
  • Flexibility to participate in an on-call rota, including evenings and weekends where required
  • A valid UK driving licence and access to a reliable vehicle is preferred
  • NVQ Level 3 in Health and Social Care or equivalent qualification is advantageous
