About the Role

We are recruiting on behalf of a busy and well-established domiciliary care and complex care provider based in Brentwood, Essex. We are looking for a dedicated and experienced Care Coordinator to join their growing team on a permanent basis. This is an excellent opportunity for a motivated individual who is passionate about delivering high-quality care services. You will play a pivotal role in coordinating care packages, managing care staff, and ensuring that service users receive the very best standard of care. The successful candidate will thrive in a fast-paced environment and will be confident in managing complex care requirements across the local area.

Responsibilities:

  • Coordinating and scheduling care visits for a large team of care workers across domiciliary and complex care packages
  • Acting as the first point of contact for service users, families, and healthcare professionals regarding care arrangements
  • Managing and monitoring care rotas to ensure full coverage and continuity of care at all times
  • Conducting care assessments, reviews, and risk assessments for service users in their own homes
  • Ensuring all care plans are up to date, accurate, and reflective of each individual's needs and preferences
  • Supporting the recruitment, induction, and ongoing supervision of care staff
  • Responding efficiently to any changes in care requirements, last-minute cancellations, or staffing issues
  • Maintaining accurate records and documentation in line with CQC regulations and company policies
  • Building and maintaining strong relationships with healthcare professionals, local authorities, and commissioning bodies
  • Participating in the on-call rota as required, with additional payment provided for on-call duties
  • Ensuring full compliance with safeguarding policies and procedures at all times
  • Supporting the Registered Manager with audits, inspections, and quality assurance activities

Ideal Candidate:

  • Previous experience working as a Care Coordinator, Senior Carer, or in a similar coordinating role within domiciliary or complex care
  • Strong knowledge of care planning, risk assessments, and CQC regulatory requirements
  • Excellent organisational and time management skills with the ability to manage multiple priorities simultaneously
  • Outstanding communication and interpersonal skills, both written and verbal
  • Ability to work effectively under pressure in a busy, fast-paced environment
  • Proficiency in care management software and scheduling systems
  • A compassionate and person-centred approach to care delivery
  • Strong problem-solving skills and the ability to make confident decisions
  • NVQ/QCF Level 3 in Health and Social Care (or equivalent) is desirable
  • A full UK driving licence and access to a vehicle is preferred
  • Flexibility to participate in the on-call rota when required

Salary: £28,000 – £31,000 per annum, depending on experience

Working Hours: Monday to Friday, 10:00am – 6:00pm
