What you’ll be doing:
An exciting role to work in an integrated health and social care team. Working within the multi-disciplinary team and applying a strength based community focused model of social care to complement the treatment regime of older adults with mental health difficulties. The social workers will manage a caseload of service users and/or their carers to undertake appropriate Care Act assessments and support them to identify their strengths and support needs. The social worker will also provide specialist support and advice to the Slough Memory Clinic.
