Responsibilities: You will be working within a community-based team and applying a strengths-based, locality-focused model of social care. Social Work Assistants will hold a caseload of residents and their informal carers, undertaking appropriate assessments of need, care and support plans and reviews in accordance with legislation, the Council and Departmental Procedures and best practice.
