Contract Type: Permanent
Working Hours: Rota working across 7 days a week including weekends, bank holidays and out of hours as required. Rotas are subject to change based on the needs of the service.
Location: Observatory House/ Slough Hybrid (minimum 2 days per week in the office). This role involves working from people's homes in the community and visiting hospitals or care home settings if required as part of the reablement interventions.

What you'll be doing:
We are seeking 4 passionate and skilled Occupational Therapists to join our Reablement and Independence Service (RIS). This community-based role focuses on empowering individuals to regain or maintain independence in their own homes through strength-based assessments, rehabilitation, and recovery-focused interventions.
Working within a multi-disciplinary team (MDT), you'll support residents with a range of needs including physical and mental health conditions, learning difficulties, and age-related challenges. You'll also collaborate with colleagues across the Local Access Point, Locality and Hospital Social work and Discharge to Assess teams to ensure seamless care transitions and effective discharge planning.
