What you’ll be doing

We’re looking for a dedicated and skilled Social Worker to join our Hospital Team. You’ll have experience in strengths‑based practice, working in a fast‑paced, pressured environment, through the Discharge to Assess (D2A) pathway. The ideal candidate will demonstrate strong assessment and care planning skills, an understanding of funding streams, and the ability to collaborate effectively within a multi‑disciplinary team. The ideal candidate will have a clear understanding of statutory duties, relevant legal frameworks, and funding streams to ensure safe, timely, and cost‑effective care planning.
