We are seeking a motivated Practice Nurse to lead and deliver high-quality respiratory care, including managing asthma and COPD in line with QOF requirements and national respiratory guidelines. The role requires independent clinical decision-making, with support from GPs and the wider multidisciplinary team when required. The successful candidate will be adaptable, proactive, and committed to delivering safe, evidence-based care.
Work collaboratively with the general practice team to meet the needs of the patients, ensuring that practice policies and procedures are followed.
Main duties of the job- Deliver comprehensive respiratory care for patients with asthma and COPD, including structured annual reviews in line with QOF and national respiratory guidelines.
- Assess, diagnose (where appropriate), and manage both acute and chronic respiratory conditions, including exacerbations, within scope of practice.
- Provide inhaler technique assessment and optimisation as well as referring to, and interpreting, spirometry/FeNO.
- Deliver comprehensive wound care management, including assessment, treatment, and ongoing monitoring and oversight of acute and chronic wounds in line with current clinical guidelines.
- Provide cervical smear screening in accordance with national screening programmes, ensuring patient comfort, safety, and accurate documentation.
- Support travel health services (where applicable), offering advice, risk assessment, and appropriate vaccinations.
- Administer childhood immunisations and vaccinations across all age groups, adhering to national immunisation schedules and best practice guidance, while promoting patient education and informed consent.

Job responsibilitiesSleaford Medical Group is seeking a motivated and proactive Practice Nurse to lead and deliver high-quality respiratory care for patients with asthma and COPD. Working within a well-established, patient-focused GP practice, the post holder will play a key role in delivering evidence-based care in line with QOF requirements and national respiratory guidelines.
The role requires clinical autonomy, with the ability to make independent decisions while working collaboratively with GPs and the wider multidisciplinary team. The successful candidate will be adaptable, forward-thinking, and committed to providing safe, effective, and patient-centred care to meet the needs of the local population.

Person SpecificationQualifications- Qualified and Registered Nurse with the NMC, with relevant asthma and COPD training and experience.
- Prescribe independently, in line with practice formulary.
- Demonstrable experience in asthma and COPD management, including delivering structured reviews.
- Experience of working within professional boundaries and recognising own limitations, with a commitment to ongoing development.
- Proven ability to communicate effectively with patients, carers, and colleagues, both verbally and in writing, and to work collaboratively within a multidisciplinary team.
- Experience of maintaining accurate and contemporaneous electronic clinical records.
- Working knowledge of, and adherence to, NHS and practice policies, including Data Protection, Confidentiality, Infection Prevention and Control, Safeguarding, and Health & Safety.
- Basic IT skills, including experience using clinical systems and Microsoft Office applications (Word, Excel, email).
- Experience of following established procedures and protocols within a clinical setting.
- Experience of delivering patient-centred care with empathy and professionalism, including supporting anxious or vulnerable patients.
- Ability to manage challenging patient interactions calmly and appropriately.
- Demonstrates insight into own competence and works safely within scope of practice.
- Experience in primary care or general practice setting.
- Experience of using SystmOne.
- Use appropriate infection control procedures, maintaining work areas in own designated clinical room so that they are clean, safe and free from hazards, reporting any potential risks.
- Understand and apply the principles of the cold chain.
- Know the health and safety policies and procedures within the workplace, including fire procedures, maintaining accurate documentation, consent, sharps and needlestick injuries.
