Salary: Β£48,000 Pro Rata + Mileage + On-Call Payments
Permanent | Part-Time (3 Days per Week) (Full-Time Considered for the Right Candidate)
An exciting opportunity has arisen for an experienced Complex Care Nurse to join a growing specialist healthcare provider delivering high-quality complex community care services to both adults and children.
Based from the Wakefield office, you'll manage a varied caseload of approximately 14 complex care packages, with the majority supporting children with complex clinical needs. Most packages are located across Wakefield, Huddersfield and Holmfirth, with occasional travel to Nottingham (typically once per week) as required.
Working alongside multidisciplinary teams, you'll play a pivotal role in ensuring outstanding clinical governance, supporting care teams, and delivering safe, person-centred care that enables individuals to live independently within their own homes.
Key Responsibilities
  • Manage a caseload of approximately 14 complex care packages, predominantly paediatric
  • Complete clinical assessments, care plans, risk assessments, monthly audits, and care package reviews
  • Provide clinical leadership and ongoing support to care teams delivering specialist care
  • Investigate incidents and support quality improvement initiatives
  • Recruit, train, mentor, and develop complex care staff alongside operational teams
  • Deliver competency assessments and clinical training programmes
  • Work collaboratively with MDTs, commissioners, hospitals, and community professionals
  • Attend regular case management and multidisciplinary meetings
  • Participate in a rotational on-call service, providing out-of-hours clinical support (additional payment provided)
  • Promote best practice and ensure high standards of clinical governance and compliance
Requirements
  • Active NMC Registration
  • Degree in Nursing
  • Full UK Driving Licence
  • Minimum of one year's post-registration experience supporting Tracheostomy and Ventilator care
  • Previous experience within Community Complex Care
  • Teaching, mentorship, or clinical training experience
  • Excellent clinical assessment, communication, and organisational skills
  • Ability to work autonomously while collaborating with multidisciplinary teams
  • Leadership or management experience is desirable
