An exciting opportunity has arisen for a Community Care Lead to join a growing provider of Complex Community Care services supporting adults and children with a wide range of complex health needs. This is a predominantly client-facing role, ideal for someone who enjoys building strong relationships while ensuring the highest standards of care are maintained within the community. Working autonomously from home, you'll manage your own diary, carry out regular client visits, support care teams and act as a key point of contact for service users and their families. Unlike many community roles, rota management and administration are managed by a dedicated support team, allowing you to focus on quality, client engagement, and service delivery.

Key Responsibilities

  • Build and maintain strong relationships with clients, families, and healthcare professionals
  • Conduct regular face-to-face client visits based on care package complexity
  • Complete staff supervisions, spot checks, and quality assurance visits
  • Support care teams to ensure safe, effective, and person-centred care
  • Respond confidently to client concerns and resolve issues professionally
  • Identify risks and escalate safeguarding or staffing concerns where appropriate
  • Work closely with operational, clinical, and bookings teams to ensure seamless care delivery
  • Maintain accurate documentation and support continuous service improvement
  • Promote high standards of care and compliance across all packages

Requirements

  • Previous experience in a Field Care Supervisor, Community Care Lead, Care Coordinator, Senior Support Worker, or similar role
  • Background within Complex Care, Community Care, Domiciliary Care, Learning Disabilities, or Mental Health
  • Previous care experience is essential
  • Confident managing challenging conversations and resolving client concerns
  • Excellent communication and relationship-building skills
  • Organised, resilient, and able to work independently
  • Full UK driving licence and willingness to travel across the North West
  • Experience within Complex Care is advantageous but not essential
