SJK Healthcare is urgently recruiting an experienced Geriatric Medicine Outpatients Nurse.
Key Responsibilities
- Supporting consultant‑led geriatric & frailty clinics
- Completing comprehensive patient assessments, including cognitive and mobility reviews
- Managing patients with common geriatric conditions such as:
- Frailty & falls risk
- Dementia & cognitive impairment
- Multiple long‑term conditions (multimorbidity)
- Polypharmacy concerns
- Mobility issues & deconditioning
- Continence issues
Requirements
- Registered Nurse (NMC PIN required)
- Minimum 1 year experience in Geriatrics, Frailty, General Medicine, or Community Nursing
- Strong understanding of elderly care pathways
- Excellent communication, compassion & patience
- Ability to manage complex cases and vulnerable patients
- Confident working both independently and as part of an MDT
- You must live in the UK and have the right to work in the UK
