Hillingdon based in West London. Social Worker needed for Adult Social Care Single Point of Access (SPA) for a busy duty team with a focus on short term and immediate intervention.
Duties include triaging concerns, dealing with cases that require an urgent or immediate response, completing duty assessments and support planning, emergency visits, cases coming in via Heathrow Airport (No Recourse to Public Funds, homelessness). This team is the point of access for all referrals to adult social care including self referrals and professional referrals. Working with partner agencies such as housing, reablement, safeguarding, mental health etc.
The candidate will need to have a wide knowledge of the Care Act, excellent organisational skills and can evidence an ability to work in a fast paced environment and skills around risk assessment and prioritisation.
