Whether you are newly qualified or an experienced nurse, this is an exciting opportunity to join our friendly and supportive team at Skylark Rehabilitation Unit. Skylark is a 30-bed community rehabilitation unit in Yate, South Gloucestershire, supporting patients on Discharge to Assess Pathway 2. We are committed to helping people regain their independence and achieve the best possible outcomes following illness, injury or surgery.

Working as part of a skilled multidisciplinary team, you will provide person-centred nursing care and rehabilitation to patients with a wide range of needs, including frailty, orthopaedic conditions, post-surgical recovery and long-term neurological conditions. Main duties of the job:

  • The unit nurse is responsible for the assessment, planning, implementation, and evaluation of care for a group of identified service users and will provide and maintain a high standard of patient-focussed care that ensures patient safety; dignity and respect are paramount to the care delivered.
  • Provide, monitor and ensure that a high standard of safe and effective nursing care is maintained via professional actions, verbal and written skills.
  • With the support of senior staff be responsible for the initial holistic assessment, implementation and evaluation of programmes of care for patients who are acutely and chronically unwell; often with complex needs arising from the frail elderly cohort.
  • Carry out a range of routine and non-routine tasks including medication rounds, clinical observations, planning, implementing, and evaluating care plans, assisting patients to meet their daily care needs, health promotion, venepuncture, planning discharges for patients.

Work Pattern:

You will be expected to work weekends and bank holidays on a rostered basis. Day and night shifts but predominantly day shifts.

Day shift: 07:00 - 19:30

Night shift: 19:00 - 07:30

Person specification:

  • Qualifications: BSc or equivalent experience in adult nursing role
  • Current NMC registration

Skills, knowledge and experience:

  • Effective communication skills written, verbal and non-verbal with the ability to adapt communication for individuals with communication difficulties
  • Able to function effectively as a team member
  • Able to make clinically reasoned judgements when working with a variety of conditions
  • Able to work autonomously, and ability to know when to seek appropriate advice / guidance from senior staff
  • Effective time management and caseload management
  • Have experience in a wide variety of nursing areas, including in the acute setting (this may be as student placements if newly qualified)
  • Able to carry out the range of professional services and technical elements in a ward including clinical assessment and record keeping.
  • Experience of the application of health, safety and risk management policies
  • Working with a range of professionals in Health or Social Care

You must have appropriate UK professional registration. This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
