Sirona's Community Homeless Health Team supports individuals who are homeless, sofa surfing and in temporary and emergency accommodation. We help 'bridge the gap' and promote re‑engagement with health care. This is an opportunity to engage with individuals facing complex health challenges, including those experiencing homelessness. You’ll deliver holistic, evidence-based care — from assessment and diagnosis to treatment and follow‑up — while working both independently and as part of a strong multi‑agency team. Main duties of the job:

  • You will contribute to tackling health inequalities by working with the most vulnerable groups.
  • You will proactively engage with individuals who have identified health inequalities at a range of venues in local communities, helping them to improve their lifestyle and overall health.
  • You will provide care for the patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.
  • You will work collaboratively with other agencies including health and social care professionals, specialised voluntary staff and Police to meet the needs of patients.
  • You will need to quickly develop trust with this cohort of patients, encouraging them to access and utilise services for their often complex needs.

Person specification:

  • Current professional registration
  • Coaching or teaching qualification (or willingness to undertake)
  • Valid driving licence & own reliable transport
  • Evidence of up-to-date clinical skills relevant to the role
  • Holistic assessment skills
  • Effective communication skills; verbal, electronic and written
  • Range of IT skills including electronic communication and clinical record systems
  • Previous community experience
