Make a difference to the community you live in and care about as a Community Support Worker within the Health, Equity & Inclusion Team. The post is based at Charlotte Keel Medical Centre and works with local communities and partners to address health inequalities and improve wellbeing outcomes.

Main duties of the job:

  • Engage with local communities to build relationships and gain insight into community strengths, needs and priorities.
  • Work collaboratively with VCSE organisations, community groups and system partners to support joined‑up approaches to health and wellbeing.
  • Support the identification and reduction of health inequalities through targeted engagement and inclusive practice.
  • Gather, interpret and feedback community insight to inform service improvement and design.
  • Contribute to preventative approaches that improve long‑term health and wellbeing outcomes.
  • Support the development of neighbourhood‑based approaches to care and wellbeing.
  • Promote inclusion, equity and accessibility across services and community engagement activities.
  • Support Sirona’s Community Appointment Days, including taking basic observations and providing advice and signposting.
  • Work independently and as part of a wider team to deliver agreed priorities and objectives.
  • Ensure all work is carried out in line with organisational values, safeguarding guidance and equality principles.

Qualifications:

  • English & Maths A‑C GCSEs or equivalent.
  • NVQ Level 3 in Health & Social Care or willingness to work towards it.

Skills, Knowledge & Experience:

  • Previous experience in a health or social care setting.
  • Understanding of data protection and confidentiality.
  • Ability to build effective working relationships with colleagues internal and external to the organisation.
  • Access to reliable transport and ability to travel independently across BNSSG.
  • Experience of working with individuals and families in a health and social care setting.
