The Community Support Worker will provide clinical and therapeutic support to patients within the community, working collaboratively with the multidisciplinary team.
Main Duties:
- Undertake delegated clinical, therapy and nursing activities for Integrated Network Team caseload.
- Provide safe, patient‑centred clinical activities in the community setting.
- Work within adult services to deliver a full range of healthcare activities.
- Collaborate flexibly with other teams or services across the locality and Sirona wide when the need arises on short notice.
- Travel throughout the service delivery geographical area as required.

- The position offers full‑time and part‑time hours with flexible working patterns, currently running from 8am – 8pm, 7 days a week.
