Clinical and Operational Lead - Community Transfer of Care Hub in Bristol

Are you a registered healthcare professional looking for an exciting leadership opportunity where you can work collaboratively with partner organisations to ensure patients access the right services following discharge from hospital? We are seeking an experienced and motivated Lead to join the Community Transfer of Care Hub (CToCH), a BNSSG-wide service providing community expertise into Southmead Hospital, Bristol Royal Infirmary and Weston General Hospital. The service is supported by community-based offices across Bristol, North Somerset and South Gloucestershire and consists of administrative staff and clinical case managers from a range of nursing and allied health professional backgrounds. The team plays a vital role in managing, navigating and co-ordinating referrals into the three Discharge to Assess pathways, ensuring patients receive the right care, in the right place, at the right time.

Main duties of the job:

• You will provide clinical leadership to the Sirona team based in the Southmead Transfer of Care Hub.
• Supporting Sirona Case Managers in reviewing patients due to be discharged on Pathway 2 and completing holistic assessments to support navigation decisions.
• You work in partnership with acute colleagues, local authorities and Sirona teams to determine the right discharge pathways.
• Representing Sirona in cross-organisational meetings.
• Liaising closely with Pathway 1 and Pathway 2 teams to problem-solve and find solutions to enable timely hospital discharges for complex patients.
• As part of our transition to becoming 'One CToCH' BNSSG service, you will provide operational line management across the team.
• This includes staff based in the South Bristol Community Hospital office.
• As part of the role you will be required to participate in Leads duty rota – acting as a point of contact for staff working across all bases; be involved in reviewing processes and on-going service developments.
• As part of the leadership team, you will need to be flexible to support other areas as required.

You will need to have excellent understanding of community services, great communication skills and be passionate about ensuring patients get the right care following a hospital admission.

Person specification:

• Qualifications: Registered healthcare professional (NMC, HCPC or equivalent) with a relevant qualification in Nursing, Occupational Therapy, Physiotherapy, Paramedic Practice or related profession.
• Experience, skills and Knowledge: Significant experience in complex case management, discharge planning and supporting patients with acute and long-term conditions. Excellent communication, influencing and relationship-building skills, with the ability to work effectively across organisational boundaries. Experience of leading, managing and developing staff, including performance management and clinical supervision. Experience of working across both acute and community healthcare settings.
