We are looking for motivated nurses who have a genuine interest in working with elderly service users and delivering high standards of care. We are looking for Nurses who are wanting to provide our patients with the best experience and treatment possible and are ready for the challenge of helping us to further develop our rehabilitation services.

You will work as part of our multidisciplinary team, all working together, to deliver individualised nursing and rehabilitation care to patients with the common goal of delivering the best outcomes possible for our patients. Our patients present with a broad range of conditions including orthopaedic, frail elderly, post-surgery and long term neurological conditions and we work with them to help them attain the highest level of personal independence prior to discharge. Whether you are a newly qualified or an experienced nurse - this is a chance to develop your skills as part of a highly supportive team.

Main duties of the job:

• The unit nurse is responsible for the assessment, planning, implementation, and evaluation of care for a group of identified service users and will provide and maintain a high standard of patient focussed care that ensures patient safety; dignity and respect are paramount to the care delivered.
• Provide, monitor and ensure that a high standard of safe and effective nursing care is maintained via professional actions, verbal and written skills.
• With the support of senior staff be responsible for the initial holistic assessment, implementation and evaluation of programmes of care for patients who are acutely and chronically unwell; often with complex needs arising from the frail elderly cohort.
• Carry out a range of routine and non-routine tasks including medication rounds, clinical observations, planning, implementing, and evaluating care plans, assisting patients to meet their daily care needs, health promotion, venepuncture, planning discharges for patients.

Work Pattern:

You will be expected to work weekends and bank holidays on a rostered basis. Day and night shifts but predominantly day shifts.

Day shift: 07:00 - 19:30

Night shift: 19:00 - 07:30

Please be aware: you will need to undertake at least one shift within a three-month rolling period to remain on the bank.

Person specification:

• Qualifications: BSc or equivalent experience in adult nursing role
• Current NMC registration
• Skills, knowledge and experience:
• Experience of working within an acute or community health care setting
• Knowledge of clinical governance and its applications
• Able to work autonomously, and ability to know when to seek appropriate advice / guidance from senior staff
• Effective time management and caseload management
• Able to reflect and critically appraise own performance
• Able to adapt communication for individuals with communication difficulties
• Up to date knowledge of evidence-based practice, local and national standards
• Working with a range of professionals in Health or Social Care
• Experience of the application of health, safety and risk management policies
• Full driving license and access to a vehicle in work time (with business use car insurance)
