Our Out of Hours Service is a crucial part of our Adults Specialist Services delivering care to people who need it urgently or those with longer-term conditions. We are recruiting Band 5 Community Nurses to help us deliver the best experience possible in South Gloucestershire, South Bristol and North Somerset. As the South West's largest provider in community healthcare, our multi-disciplinary teams of nurses, therapists, clinicians and support colleagues help people remain safe and live as independently as possible after injury, illness or hospital admission. We take a holistic approach involving people in their care, enabling them to thrive in their homes and local communities.

We work on a buddy system which means you will never be working alone; you will always have someone with you for every visit during your shift. In this role, you will focus on collaborative, safe, effective and evidenced-based practice. Working closely with primary and community services as well as with social care partners, you play a critical role in ensuring a person-centred experience. Plus, with no rush hour traffic to contend with, save time on your commute, giving you more free time and flexibility during the day for a better work/life balance. Main duties of the job

As an OOH Nurse, you will be required to:

  • Assess, plan, implement and evaluate individual, patient-centred interventions drawing on a wide range of specialist knowledge gained from practical experience and specific training.
  • Be competent in a broad range of clinical skills including catheterisation, pain management, wound care, end of life care and IV medication after specific training and more.
  • Actively use and promote shared decision-making, empowering people to improve self-management of their long-term condition while working collaboratively with our teams and system partners.
  • Supervise colleagues including peers, students and unregistered workforce in nursing competencies, helping us shape our existing service and contribute to the future design and delivery.

Person specification

Qualifications:

  • BSc or equivalent experience in adult nursing
  • Current NMC registration
  • Evidence of continuing professional development and commitment to lifelong learning

Skills, experience and knowledge:

  • Effective communication skills written, verbal and non-verbal
  • Able to adapt communication for individuals with communication difficulties
  • Able to function effectively as a team member
  • Able to make clinically reasoned judgements when working with a variety of conditions
  • Able to work autonomously, and ability to know when to seek appropriate advice/guidance from senior staff
  • Effective time management and caseload management
  • Able to reflect and critically appraise own performance
  • Up to date knowledge of Evidence Based Practice, local and national standards
  • Experience of the application of health, safety and risk management policies
  • Working with a range of professionals in Health or Social Care Community work (either student or postgraduate) demonstrating a broad range of clinical undergraduate experience

You must have appropriate UK professional registration. This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
