Sirona Care & Health C.I.C. in Bristol is seeking a Band 5 Occupational Therapist to join our Integrated Neighbourhood Team (INT).
The role focuses on delivering community-based rehabilitation in patients' homes and supporting hospital discharges to promote independent living. You will assess, plan and implement care with the MDT, supervise junior staff, arrange equipment, maintain records, and participate in training.
