Job summary

We are seeking a Nurse or Paramedic to join our team! NHS@Home is an innovative service, providing care for people who are acutely unwell in their own homes across Bristol, North Somerset, and South Gloucestershire. It offers an alternative to hospital and helps people return home from hospital more quickly. The service uses a mixture of face‑to‑face visits, telephone calls, and remote monitoring through digital technology to help people recover safely in the comfort of their own homes. NHS@Home has pathways covering a wide range of conditions, including frailty, respiratory, heart failure, OPAT, infection, and general surgical recovery. Our team provides a wide range of acute healthcare treatments within the home, in addition to monitoring health conditions and vital signs for deterioration. The service is run in partnership between Sirona Care & Health, North Bristol Trust, University Hospitals Bristol, and Weston NHS Foundation Trust.

Main duties of the job

  • Provide clinical face‑to‑face and virtual monitoring and assessment of patients in the @home service, advising on care and treatment and reporting changes in patient condition as required in line with local protocols and guidance.
  • Provide acute care to patients (in their homes) with complex and specialist needs in the community across Bristol, South Gloucestershire, and North Somerset.
  • Work alongside the multidisciplinary team to develop, implement, and evaluate care plans.
  • Assess the health and psychosocial needs of the patient within the hospital and community setting.
  • Plan and implement treatment and use a wide range of service to meet their needs.
  • Utilise a wide range of clinical skills to support the ongoing management of patients in their home environment, e.g. comprehensive health assessment, observations, venepuncture, ECG, bladder scanning and wound care.
  • Communicate effectively, network and liaise to ensure high‑quality responsive, safe, efficient and compassionate care for patients, carers, and families.

Person Specification

Qualifications

  • Degree qualification or equivalent
  • NMC Registration
  • Willingness to undertake training required for the role

Skills / Experience / Knowledge

Essential

  • Understanding of clinical governance processes with examples
  • Ability to carry out complex health assessments, including venepuncture, observations, bladder scanning, ECG & wound care
  • Ability and evidence to use Microsoft packages including Outlook, MS Teams, Excel and electronic patient records
  • Experience & ability to work autonomously
  • Experience & ability to understand knowledge base and to escalate if needed
  • Evidence of continuous personal/professional development
  • Adaptability and flexibility for effective time management within conflicting priorities

Desirable

  • Experience of training and supporting others
  • Experience of digital consultations
  • Experience of community care
  • Experience of using EMIS
  • Patient Assessment & Clinical Reasoning Course or similar

Other

Essential

  • Full driving licence
  • Use of own car with business insurance and ability to travel regularly across the BNSSG area
