Single Homeless Project is seeking a Homeless Pathway Coordinator to support patients experiencing homelessness across Camden and Newham. The role is a full-time, fixed-term contract for 12 months, with a starting salary of £34,127.11 per year and rising to £36,947.49 pro-rata as you progress.
You’ll work on wards and in A&E, navigating health, housing and support services, building trusting relationships, and ensuring the patient voice stays central in every decision.
