We are currently looking for Support Worker to provides person-centered care and practical support to individuals who may be elderly, disabled, or have physical or mental health needs.
Key responsibilities include:
Supporting individuals with personal care, daily living activities, and mobility.
Administering or supporting medication in line with care plans and policies.
Promoting independence, dignity, choice, and wellbeing.
Monitoring and reporting changes in physical or mental health.
Supporting individuals with appointments, activities, and community involvement.
Maintaining accurate care records and following care plans.
Safeguarding individuals from abuse, neglect, and harm.
Working closely with families, nurses, social workers, and other healthcare professionals.
Maintaining confidentiality and following health, safety, and infection-control procedures.
