Patient assessment: Assessing patients' physical, psychological and social health needs.
Home nursing visits: Providing nursing care to patients in their own homes, residential homes and community clinics.
Wound care: Managing wounds, pressure ulcers, dressing changes and post-operative wound care.
Medication management: Administering prescribed medication, injections and monitoring treatment effectiveness.
Chronic disease management: Supporting patients with diabetes, hypertension, respiratory conditions and other long-term illnesses.
Patient monitoring: Observing patients' conditions, identifying deterioration and escalating concerns appropriately.
Palliative and end-of-life care: Supporting patients with life-limiting illnesses and providing compassionate end-of-life care.
Health promotion: Educating patients and families about healthy lifestyles, disease prevention and self-care.
Care planning: Developing, implementing and evaluating individualised nursing care plans.
Multidisciplinary working: Collaborating with GPs, district nurses, physiotherapists, social workers and other healthcare professionals.
Documentation: Maintaining accurate patient records and completing clinical documentation.
Safeguarding: Identifying and reporting concerns involving vulnerable adults and children.
Clinical assessment, assessing patients and identifying healthcare needs.Wound management, performing wound dressing and pressure ulcer prevention.Medication administration, safely administering prescribed medicines.Clinical decision-making, recognizing deterioration and escalating concerns.Communication, communicating effectively with patients, families and colleagues.Care planning, developing and reviewing individualized care plans.Infection prevention, maintaining hygiene and infection control standards.Time management, managing home visits and competing clinical priorities.Teamwork, working effectively within multidisciplinary teams.Documentation, maintaining accurate and confidential patient records.Compassion, Providing respectful, person-centered nursing care.Safeguarding, protecting vulnerable adults and children from harm.Qualifications and professional requirementsCurrent registration with the Nursing and Midwifery Council (NMC).Knowledge of community nursing practices and procedures.Understanding of infection prevention and control.Knowledge of medication administration and patient safety.Basic life support training.Experience in clinical nursing, ideally community or acute care.Ability to work independently and make appropriate clinical decisions.Full UK driving licence is required for community visits.
