We are currently looking for Support Worker to provide person-centered care and practical support to individuals who may be elderly, disabled, or have physical or mental health needs.
Key responsibilities include:
- Supporting individuals with personal care, daily living activities, and mobility.
- Administering or supporting medication in line with care plans and policies.
- Promoting independence, dignity, choice, and wellbeing.
- Monitoring and reporting changes in physical or mental health.
- Supporting individuals with appointments, activities, and community involvement.
- Maintaining accurate care records and following care plans.
- Safeguarding individuals from abuse, neglect, and harm.
- Working closely with families, nurses, social workers, and other healthcare professionals.
- Maintaining confidentiality and following health, safety, and infection
