We are seeking a passionate, experienced, and highly skilled Advanced Clinical Practitioner (ACP) to join our dedicated healthcare team. Responsibilities

• Deliver advanced clinical care to patients, including assessment, diagnosis, planning, treatment, and follow-up across a variety of care settings.
• Act as an autonomous practitioner, using advanced clinical skills and decision-making to serve the needs of patients and their families.
• Work collaboratively with multidisciplinary teams, ensuring care is aligned with organizational policies and best practices.
• Prescribe medications, treatments, and therapies in accordance with relevant guidelines and legal frameworks.
• Provide leadership and mentorship to nursing staff, trainee ACPs, and other healthcare professionals to enhance overall team performance.
• Participate in and contribute to clinical audits, quality improvement initiatives, and evidence-based practice to ensure safe, effective, and efficient care delivery.
• Promote and support inclusive healthcare practices, ensuring the unique needs of patients from diverse cultural, social, and economic backgrounds are respected and addressed.
• Engage in continuous professional development (CPD) to maintain and enhance advanced clinical knowledge and skills.
• Promote health education and preventative care approaches in interactions with patients, families, and caregivers.

Qualifications

• Registered Nurse with active registration on the Nursing and Midwifery Council (NMC) register.
• Completion of a recognized Advanced Clinical Practitioner training program (Master's level qualification or equivalent).
• Extensive clinical experience in a relevant healthcare setting, with demonstrable skills in advanced clinical assessments and decision-making.
• Independent Non-Medical Prescriber qualification.
• Comprehensive understanding of current national and local healthcare policies, clinical guidelines, and legal frameworks.
• Strong interpersonal and communication skills, demonstrating the ability to connect with patients, families, and colleagues from diverse backgrounds.
• Commitment to upholding equality, diversity, and inclusion in all aspects of clinical practice.

Desirable:

• Experience teaching and mentoring other healthcare professionals.
• Additional specialty training or certification in one or more clinical areas.

Day-to-Day

• Conduct comprehensive clinical assessments and develop individualized care plans tailored to meet patient needs.
• Work alongside other healthcare professionals to deliver coordinated and integrated care.
• Use advanced diagnostic tools and evidence-based interventions to manage acute and chronic conditions.
• Participate in clinical ward rounds, patient meetings, and case reviews.
• Lead on patient education initiatives, providing advice and guidance to improve health outcomes.
• Review and interpret diagnostic test results to identify next steps in treatment plans.
• Collaborate with nursing staff and other clinicians to ensure quality care delivery.
• Document patient care timely and accurately in accordance with policies and standards.
• Regularly contribute to team meetings, offering input on protocols, procedures, and patient care strategies.
