- Manage chronic disease conditions such as diabetes, asthma, and hypertension by offering advice and monitoring health plans.
- Collaborate effectively with GPs, other nurses, and allied healthcare professionals to deliver coordinated patient care.
- Provide patient education on self-care, lifestyle changes, and management of health conditions.
- Maintain accurate and up-to-date patient records, adhering to confidentiality and data protection policies.
- Participate in quality improvement initiatives and assist in the development of practice policies and protocols.

Qualifications
- Registered Nurse (RN) or equivalent with a valid nursing license.
- Previous experience as a Practice Nurse or relevant clinical nursing experience.
- Strong clinical skills, including wound care, vaccination, and chronic disease management.
- Excellent communication and interpersonal skills with a patient-centered approach to care.
- Ability to work collaboratively in a multidisciplinary team as well as independently.
- Proficiency in electronic health records and basic computer literacy.
- Commitment to maintaining patient confidentiality and adhering to professional standards.
- Additional qualifications or certifications in areas such as diabetes management, sexual health, or smoking cessation are highly desirable but not mandatory.
