Responsibilities

  • Conducting comprehensive patient assessments via virtual or telemedicine platforms, ensuring accurate diagnosis and care plans.
  • Delivering advanced clinical care and decision-making in collaboration with multi-disciplinary teams to manage both acute and chronic conditions.
  • Prescribing medications, treatments, and therapies in accordance with evidence-based guidelines and within your scope of practice.
  • Utilizing advanced clinical knowledge to educate and empower patients about their health, preventive measures, and symptom management.
  • Providing leadership in clinical settings by contributing to training, mentoring, and supporting healthcare professionals.
  • Staying up-to-date with current medical advancements, best practices, and telehealth technologies.
  • Adhering to policies, procedures, and regulatory standards to ensure high-quality care delivery.

Qualifications

  • Education & Certification: Master's degree in Advanced Practice Nursing or related field. Current and active certification as an Advanced Clinical Practitioner (e.g., NP, CNS, or equivalent). Valid state licensure or eligibility for licensure within your practicing region.
  • Experience: Minimum of 2 years of experience in a clinical setting as an ACP. Strong background in managing acute and chronic health conditions. Experience working in telehealth or remote care settings is a plus.
  • Key Skills: Proficient in clinical decision-making and evidence-based practice. Strong interpersonal and communication skills to engage with patients from diverse backgrounds. Ability to manage multiple tasks/workloads in a remote environment. Competent in using telemedicine software, electronic health records (EHR), and other relevant technology.
