We are seeking an experienced Locum Diabetes Specialist Practice Nurse to support GP surgeries and Primary Care Networks across Wakefield. This role offers flexible working days and hours, with opportunities ranging from ad hoc sessions to full-time placements. The successful candidate will deliver high-quality, evidence-based diabetes care, supporting patients with Type 1 and Type 2 diabetes to optimise their health outcomes in line with NICE guidelines and QOF standards. Experience using SystmOne and working within GP practice settings is essential.

Key Responsibilities
  • Conduct comprehensive diabetes annual reviews
  • Manage patients with Type 1 and Type 2 diabetes
  • Support insulin initiation and titration
  • Optimise oral hypoglycaemic and injectable therapies
  • Monitor and interpret HbA1c, lipid profiles, and blood pressure
  • Provide structured diabetes education and lifestyle advice
  • Complete diabetic foot checks and risk assessments
  • Identify and manage diabetes-related complications
  • Develop and implement individualised care plans
  • Ensure accurate and timely documentation using SystmOne
  • Support practices in achieving QOF targets and chronic disease management goals
  • Work collaboratively with GPs, pharmacists, dietitians, and the wider MDT
Essential Requirements
  • Registered Nurse with valid NMC registration
  • Proven experience as a Diabetes Specialist Nurse or Practice Nurse in Primary Care
  • Strong working knowledge of SystmOne
  • Experience conducting diabetes annual reviews independently
  • Competence in insulin management and diabetes medication optimisation
  • Active NHS Smartcard
  • Excellent communication and patient education skills
  • Ability to work independently in GP practice environments
Desirable
  • Previous locum GP practice experience
  • Non-Medical Prescribing qualification (V300)
  • Experience working across multiple GP practices or PCNs
  • Experience supporting QOF achievement
