Main duties of the job
Key Responsibilities:
- Assist with the delivery of high-quality patient care in a GP surgery setting as part of a Nursing Team.
- Manage chronic disease clinics E.g. Diabetes, Asthma, COPD, & Hypertension etc.
- Conduct health screenings, vaccinations, and other health promotion activities.
- Managing women's health, such as conducting cytology screening & contraception assessments.
- Provide wound care, minor injury treatment, and patient assessments.
- Offer advice and education to patients on lifestyle changes and disease management.
- Work closely with GPs and other healthcare professionals to provide comprehensive care.
- Maintain accurate and confidential patient records.
- Flexibility in provision of care by recognising and prioritising health issues and intervening appropriately.
- Support the Practice Team in the event of clinical emergencies.
- Maintaining up to date evidence-based knowledge and help support the development of others in the team.
Job responsibilities
- Participate in local initiatives to enhance service delivery and patient care.
- Support and participate in shared learning within the practice in order to improve patient care.
- Continually review clinical practices, responding to national policies and initiatives where appropriate.
- Participate in the review of significant and near-miss events applying a structured approach, i.e., root cause analysis (RCA).
- Support the practice team with the reviewing and implementation of practice policies and protocols.

- Registered Nurse (Nursing and Midwifery Council)
- Mentor in teaching qualification
Experience
- Experience of working autonomously
- Experience of quality initiatives, i.e., benchmarking
Clinical knowledge and skills
- Clinical knowledge in the following:
- Wound care/removal of sutures and staples
- ECGs
- Venepuncture
Skills:
- Excellent communication skills (written and oral)
- Effective time management (planning and organising)
- Ability to work as a team member and autonomously
- Good interpersonal skills
- Ability to follow clinical policy and procedure
- Clear, polite telephone manner
Other requirements
- Flexibility to work outside of core surgery hours
- Occupational Health clearance
- Meet the requirements and produce evidence for nurse revalidation
- Evidence of continuing professional development (CPD) commensurate with the role of a Practice Nurse
- Experience with audit and able to lead audit programmes
- Experience with clinical risk management
