Office Working: Hybrid – 3 days in the office, 2 days working from home/out on visits

We are currently working with the London Borough of Bexley to find an experienced Qualified Social Worker for their Transitions and Leaving Care Service. This is an excellent opportunity to support young people in care aged 16 and 17 as they prepare for independence, ensuring they have the right plans, support and opportunities in place for a successful transition into adulthood.

This is a case-holding role where you'll manage statutory responsibilities for young people in care, completing assessments and developing comprehensive, person-centred pathway plans before they transfer to a Personal Advisor within the service. You'll work closely with young people to build their confidence and independence while supporting them with accommodation, education, employment, health and their wider aspirations.

Duties:

  • Managing a caseload of 16 and 17-year-old young people in care as they prepare for independence.
  • Completing assessments and developing high-quality, person-centred pathway plans.
  • Undertaking regular statutory visits and reviews to monitor progress, risk and support needs.
  • Supporting young people to access suitable accommodation, education, training, employment and health services.
  • Ensuring the voice, wishes and aspirations of young people are central to assessment and planning.
  • Working collaboratively with housing, education, health and community organisations to provide coordinated support.
  • Advocating for young people's rights and empowering them to develop the skills and confidence needed for adulthood.
  • Preparing young people for a successful transfer to their Personal Advisor within the Leaving Care Service.
  • Maintaining accurate and timely case records, reports and statutory documentation.

What you need:

  • Social Work Qualification.
  • Social Work England registration.
  • Enhanced DBS (provided by us).
  • Experience working with young people in care and/or within Leaving Care or Transitions services.
  • Strong safeguarding experience and knowledge of statutory responsibilities for young people in care.
  • Knowledge of relevant Leaving Care legislation, policies and procedures.
  • Ability to build positive relationships with young people while managing a statutory caseload.
  • Able to attend the office 3 days per week.
  • Strong knowledge of Child Protection, Child in Need, legal planning and court work.
  • Experience leading and developing high-performing social work teams.
